A company is looking for a Transition of Care Coordinator -
Eastern Time Zone.
Key Responsibilities
Engage and enroll patients through telephonic outreach, serving as
their primary contact
Develop and implement comprehensive care plans in collaboration
with the interdisciplinary care team
Identify and address social determinants of health, connecting
patients to necessary resources and support
Required Qualifications
3 years of experience in healthcare coordination or community
health work
Experience with a team-based approach to care
Strong knowledge of community resources
Intermediate skills with MS Office Suite, including Outlook and
Teams
Ability to work effectively in a remote environment with required
internet speeds
