Requirements: Required Certifications: BLS, MAB Required Skills: Short Term Acute Care Inpatient Medical Social Worker, Biopsychosocial assessment, Conduct initial evaluation, Coordinates with internal and external team members, Participates in care conference activities, Refers to other disciplines/resources, Discharge Summary, Documentation according to plan of care, Documents patient response, Facilitate discharge and contingency planning, Individual counseling, Educates patient/caregiver to facilitate discharge, Advance Directives, Child protective services, Adoption / Foster Care, Advance Directives, High risk intervention patient vulnerability or safety risk, Knowledge of cultural issues, Work with multi-disciplinary team to ensure continuity of care, Make referrals to professional and community services Job Details: Pay: $2130/week - $2360/week Assignment Length: 13-week assignment Shift: 5, 8-Hour 14:00 - 22:30 At least one year of experience required Aya delivers: The most jobs in the industry.
