Registered Nurse (RN) | Case Manager Location: Mission Hills, CA Agency: Cynet Health Pay: $2,691 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/2/2026 About the Position Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 2 years of acute care hospital case manager experience Must have experience in care coordination and discharge planning License: CA state license required Pending license accepted, but must be licensed upon start date Certifications: N/A Must-Have: Ability to assess patients and coordinate care Knowledge of treatment options for drug and alcohol abuse Experience working with patients experiencing homelessness and those with a mental health history Ability to organize and participate in family meetings Competence in arranging durable medical equipment and transportation Description: The position requires managing hospital case management activities. The role will involve care coordination and discharge planning. Candidates should have experience working in an acute care setting. They must be able to perform tasks independently and ensure patient needs are met. Understanding of the interdisciplinary support structure is essential. Experience with developing treatment plans and utilizing assessment criteria is required. Candidates will be expected to work on weekends, as needed by the department.
