Job Description:
A company is looking for a Nurse Case Manager.
Key Responsibilities
Develop individualized care plans to improve member outcomes and
promote wellness
Conduct comprehensive assessments and coordinate referrals to
resources
Collaborate with interdisciplinary teams to resolve care barriers
and present cases as needed
Required Qualifications
Active Registered Nurse (RN) license in Pennsylvania and New
York
3-5 years of clinical practical experience
2-3 years of experience in case management or home health care
coordination
Ability to work independently in a remote setting
Proficient with Microsoft Office applications
