Role Overview:
As a Clinical Operations Manager overseeing Care Management
Services, you will lead, manage, and hold your team accountable for
delivering high-quality chronic care coordination. You will be
responsible for implementing efficient workflows, driving
productivity, ensuring quality assurance, and collaborating with
partners to achieve clinical and operational excellence.
Key Responsibilities:
Lead, manage, and hold care management team members accountable for
performance and outcomes.
Streamline and improve operational efficiencies for care management
service lines.
Implement new service lines and workflows in collaboration with
internal and external stakeholders.
Monitor team productivity and drive continuous improvement through
data-driven decision-making.
Conduct quality assurance reviews to uphold clinical
excellence.
Provide training, resources, and ongoing coaching to the care
management team.
Improve patient connection rates using innovative tools and
strategies.
Collaborate with the Partner Success & Growth Coordinator to
support partner initiatives.
Attend partner meetings to ensure alignment and advocate for
operational success.
Utilize the Team Performance Assessment Tool for weekly performance
evaluations and accountability tracking.
